GP Extended Access - Public Survey

The six NHS Clinical Commissioning Groups across Staffordshire and Stoke-on-Trent are looking at how to give you easier and more convenient access to services you have at your local family doctor and GP surgery available outside of the usual surgery hours, including over the weekends.

We are interested in hearing your views about how, when and where you might choose to use these services outside of the usual surgery hours.

This survey will remain open until Thursday 8th February 2018 and the information gathered will then be reviewed with current and potential providers of these services. Patient representatives will be involved in this work and in developing options for each area of Staffordshire and Stoke-on-Trent.

Data Protection

The information you supply will be stored and processed by Midlands and Lancashire Commissioning Support Unit (MLCSU) in accordance with the Data Protection Act 1998. It will be used to analyse the questionnaire responses, check the analysis is fair and accurate, and help us to gather feedback more effectively in the future.

Any reports published using this information will not contain any personally identifiable information. We will provide anonymised and aggregated responses to North Staffs Clinical Commissioning Group & Stoke Clinical Commissioning Group, East Staffordshire Clinical Commissioning Group, South East & Seisdon Clinical Commissioning Group, Cannock Clinical Commissioning Group and Stafford & Surrounds Clinical Commissioning Group.
